2025 Medicare Part D Prescription Payment Plans
For 2025, Medicare Part D plan members will be able to opt into a prescription payment plan to distribute the cost of their drugs over the course of the year. It is focused on people who have high drug cost where they must meet a drug deductible. It is not a good option for many people.

      Depending on the drone and considering line-of-sight communication, there is a point on the northern side of the canyon that could work for launching a drone. A better option might be on the Placer County side of the river, directly in front of the canyon. Either location requires some hiking in tough terrain. If the lake is low enough, you can skirt along the edge of the lake/river from the Birdsall location. The best time would be either fall, after a rainstorm so the waterfalls are gushing with water, or in the spring.

    I will be covered under Medicare next August. Currently, I am covered thru covered California MediCal under MAGI due to no income and my eligibility just got approved as my anniversary is September. I don’t plan on continuing with MediCal once I get under the Medicare coverage. I will also be signing up for Social Security starting August 2025 which will anyways put me over the FPL income together with my 401k withdrawals. At the time when I get covered under Medicare, do i just call MedICal to let them know I no longer need coverage anymore and is that all I have to do? I also will not need the Medi-Medi option as I will be able to pay my supplemental and Medicare premiums from the social security income plus 401 K. Will stopping medical at that time cause be any issues with Medicare premiums deductions? I will want to be only on Medicare with supplemental starting August next year with no Medical anymore or also no Medi-Medi anymore as I will be financially able to pay my premiums. Please advise me how to successfully terminate MediCal under MAGI with no issues and smoothly transition into full Original Medicare with supplemental and allow my premiums to be deducted from the SS check. Is there anything I need to let SS department know that I no longer will be covered under MedICal once Medicare kicks in. Sorry for the long comment but I just want to do it the right way. Ending one and starting the other on the right foot.

    • @KevinKnauss
      @KevinKnauss 6 днів тому

      Medi-Cal will get notified of your impending transition to Medicare. They will try to keep you in the system, that's their job. You will need to communicate with them regarding your projected income from SS and 401K distribution. You will need to have proof. The SS award letter is one form of proof. If SS is enough to lift you beyond the Medicare Savings Program (aka Medi-Cal and Medicare) that could be enough. The county can be tenacious. I just had a client who was flipped into Medi-Cal for her and her Medicare spouse. They make far too much for any program. However, the county Medi-Cal case worker either did not understand their income statement or did not believe it because they did not submit the proper documents. If you start SS and 401K distributions after you enroll in Medicare, you will be transitioned into the Medi-Cal extra help system. That does not preclude you from enrolling in any Medicare Advantage, Medicare Supplement, or Part D plan you want. If you want to avoid the Medi-Cal drama, you will need to trigger an increase of income before you are Medicare eligible so all of those documents can be forwarded to Medi-Cal and they will not consider you eligible.

    • @debbiea1723
      @debbiea1723 6 днів тому

      So you are suggesting that I start my 401k withdrawals and SS withdrawals earlier than my Medicare eligibility but I don’t want to lose medical until August next year. They would kick me off medical earlier than I need to due to income changes. I was planning on doing SS and 401k withdrawals and Medicare all in August next year when my Medicare coverage begins. I will need Medical until then. Do you recommend I start my 401k and SS after I become Medicare eligible

      @@debbiea1723 I’m not suggesting anything. I’m just giving you the course of events. If you have $0 income when your turn 65, you will most likely be Medi-Medi. That is not a bad thing. When your income increases because of the SS or 401k distribution, report that to Medi-Cal and you will lose the extra help.

      Technically, you could lose it for one month. However, they try to spread those one-time lump sums over the year, so you maintain eligibility. I'd report it as soon as it comes in so the county can properly classify it to help you maintain your eligibility for the programs you qualify for.

      You won't count your baby, but Medi-Cal will figure it into the equation. For example, pregnant women can qualify for the Medi-Cal for Pregnant individuals up to 213% of the federal poverty level. Normally, for adults, you have to earn less, 138% FPL to qualify for Medi-Cal. To your point, you just have to notify Medi-Cal of your situation, and they will take care of the rest.

    Thanks for the video. I am retiring early from my job and my coverage ends on Sept. 31st. I will need coverage from Oct - Dec. My job has been covering our family for medical. My husband is offered medical at his work but it is very expensive. Can we still get covered california if it is cheaper than what his will cost? Also, even if I don't get a large subsidy for 2024 does it make sense to get it now based on the special event and that way we can continue it in open enrollment but get a much larger subsidy because we will have less income. Thanks again.

    • @KevinKnauss
      @KevinKnauss 16 днів тому

      You are asking all the right questions. New rules allow family members to get the subsidies when one of the household members is offered employer sponsored health insurance. The calculations are a little cumbersome. This post goes through the step with the caveat that the household percentage has changed for 2024, I think it is 9.2%. insuremekevin.com/relief-for-crippling-employer-health-insurance-premiums-covered-california/#gsc.tab=0 The other path is waiting for open enrollment for 2025 when your estimated income will be lower because you won't have the employment income. That should translate into a larger subsidy. You will still have to crunch the numbers to see if you are eligible for Covered California subsidies with the other employer sponsored health insurance. Unfortunately, you will need to do the math on a spreadsheet to see if any of the scenarios make financial sense. The other issue is the health insurance and if any of the health plans include your preferred providers. It is possible for your spouse to cover some of the dependents on the group plan while other household members go through Covered California. Some people do this to keep certain doctors because a family member may have a health challenge or is going through treatment.

      @@KevinKnauss Thank you for the info. I was thinking that maybe I should have my daughter (19) go on medicale and my son (24) go on his own covered california policy - both file their own taxes and then just cover my husband and myself on cobra for the remainder of the year and then go on covered california. Does that sound like it would be the cheapest? Am I allowed to do all of that? Thank you so much for helping me.

    • @KevinKnauss
      @KevinKnauss 16 днів тому

      @@TrishaRico Good alternatives. The dependents have QLE of Loss of Coverage and can apply through CC to see if they are Medi-Cal or subsidy eligible. Just remember that you cannot take them as dependents on your federal tax return for 2024 if they are Medi-Cal or subsidy eligible with their own health plans. You and your spouse can remain on COBRA through Dec. 31. During open enrollment you can apply for CC and subsidies for 2025 if your coverage under the employer plan makes the health insurance premium more than the set affordable percentage (9.2%?) Note, the affordability is based on the least expensive health plan offered to the employee, not necessarily the plan the employee has enrolled into.

      @@KevinKnauss so from what I understand you saying is that in 2025 when it is just my husband and myself and our income is say 70k our covered california would be $216 mth but his cheapest medical plan for the two of us would be $950 mth then I can get the covered california because it is over the 9.2% of what covered california would cost????? @KevinKnauss 16 днів тому

      @@TrishaRico I'm not sure of the exact household percentage, but if the total monthly premium for all household members is greater than the percentage (9.2%?), then dependents can get the subsidy from Covered California. The employee, whose premium is usually reduced by an employer contribution is not eligible for the subsidy because their premium is considered affordable. You would enroll through CC with a household of 2, one opting out of the health plan because. The remaining household member would then be offered a subsidy.

      If you are a single individual, the calculated income for Qualified Medicare Beneficiary is $1,275 per month. For Specified Low Income Medicare Beneficiary, the income must be under $1,526. • @KevinKnauss
      @KevinKnauss 22 дні тому

      Two scenarios. 1. You can keep Covered California, but technically you may not be eligible for the subsidies because of being offered affordable employer group health insurance. Spouse can be removed from health plan, you can keep it with no subsidies. 2. There is a possibility of you keeping the subsidies IF the household employer group Kaiser premiums is greater than approximately 9.2% of the household income (% changes every year.) Kaiser plan may be affordable for spouse, but not the household. Usually the employer makes a contribution to employee, not spouse or dependents. It’s complicated, but I have a couple of clients who verified they are eligible for the CC subsidies while the spouse stayed in the group plan. Given the removal of the asset limit for most Medi-Cal programs in 2024, my understanding is that an inheritance only affects Medi-Cal eligibility to the extent that it raises your income for the month the inheritance is received (assuming it is a single lump sum), and the extent of any future earnings on the inherited assets. So if a person currently has an income that qualifies them for Medi-Cal QMB or A&D FPL, and receives a $100k inheritance, and reports it within 10 days, then I assume they will lose their Medi-Cal benefits. If we assume that the next month their income going back to what it was before, should they be able to start getting Medi-cal benefits again immediately, or perhaps after ~3 months given delays in processing?

    • @KevinKnauss
      @KevinKnauss 24 дні тому

      My understanding is that an inheritance would be consider an increase to your assets, not necessarily income. Medi-Cal does understand that little bumps of money can create a seesaw effect where people are on and off of Medi-Cal, very disruptive. They look to smooth out fluctuating income streams so people are not thrown off Medi-Cal every other month. Most people are re-qualified annually if the individual has both Medicare and Medi-Cal. If the inheritance is considered income and reported to the Social Security, the individual may not be eligible for Medi-Cal the next year.

    • @KevinKnauss
      @KevinKnauss 25 днів тому

      Covered California might be less expensive, if it is available to you. Technically, unless you are within your 60-day Special Enrollment Period window, triggered by a loss of coverage, then Covered California is not an option in most situations. The household income is another consideration. If the household income is very high, or was very high previous to the separation from the employer, then there might not be much subsidy to help reduce the health insurance premiums. Other factors are where you live and the ages of the household members. Older adults in Northern California almost always get subsidy help, even with very high incomes. If you are younger in Southern California, the subsidies are smaller because the health insurance premiums are lower in Southern California.

With this new rule, can I transfer what's left in my trust account to my personal account? Thank you.

    • @KevinKnauss
      @KevinKnauss 27 днів тому

      Technically, I would say yes. But you should consult a tax professional or lawyer about whether the transfer will create a taxable income event. That could complicate the situation. If it is a large amount, and it is taxable, it will show up at Social Security. There may be other options such as funding a retirement account. This would be akin to rolling over 401K money into an IRA. When done properly, there are no taxes on the transfer. But if you take possession of the money, then it can be considered taxable income.

    • @KevinKnauss
      @KevinKnauss Місяць тому

      The subsidy is calculated on the household size from the tax return. If dependents in a 4 person household are Medi-Cal, based on income, removing the dependents with the same income, the adults would continue on Covered California. The income necessary to qualify for Covered California increases with household size. This is assuming that the dependent income is not being entered for the household income. Potentially removing dependents with income could drop the adults into Medi-Cal.

      Covered California must defer to Medi-Cal eligibility determination. Once you are Medi-Cal eligible, or anyone in the household is Medi-Cal eligible, everything must go through your county Medi-Cal office. If you had been a Kaiser member in the last 6 months, and are Medi-Cal eligible, you are supposed to be able to select Kaiser as you Medi-Cal HMO plan. The only way to get back to CC is to prove you are not Medi-Cal eligible. However, the system is super slow and customer service is not a term the Medi-Cal uses. They are understaffed and over worked. I would suggest setting up a Benefitscal.com account, if you have don't already have one. That seems to have worked for many people to communicate with Medi-Cal.

    Hello Kevin. I have a lengthy paragraph ahead and I’m very confused and how you can clarify. Currently a family of 5 making 73k on medi-cal. Renewal is coming up, based on the income chart we will be pushed over to covered California. Ages in the family are 54.42.19.17 and 9. Will our household still be able to qualify for medical on other ways than income. In the dchs website I am seeing anyone below 21 or 26 is eligible for age based medical. Which one is the actual age? 21 or 26? I’m seeing both. Then I’m seeing if you are a parent of an age eligible child, you also qualify. So based on this info does this mean my household will have all 5 people renewed for medical based on these qualifications other than income. This is the first year the our income has passed the 138% mark due to the 19 and 17 yr olds getting part time jobs. Hope you can clarify thanks.

    • @KevinKnauss
      @KevinKnauss Місяць тому

      In general, if you are 19+ years old, and the household income is over 138% FPL, you are Covered California eligible. All that stuff about parents of an eligible child being Medi-Cal relates to the child or dependent being disabled. What is more crucial is your tax household, who is being claimed as a dependent, and the part-time income. If the income of the dependent is below the tax filing threshold - they file a tax return to get the withholding returned and owe no income tax - that income is not counted in the household income. Sometimes, young adults are better off not being in the parent’s household. That is something to consider if taking the young adult as a tax dependent is borderline.

I had two county workers each gave me different info, one said during last year renewal that we were all eligible for medical, and one told me just few days ago that both us adults are no longer eligible even no change in incomes. Could you tell us, are we eligible or not?

    • @KevinKnauss
      @KevinKnauss Місяць тому

      Based on the February 2024 Covered California income table, for a family of 5, at $5,773 monthly income, the adults would be eligible for Covered California and the dependent children would be Medi-Cal eligible. The threshold for adult Medi-Cal in a household of 5 is a monthly income under $4,208 per month, $50,481 annually. It is possible that last year, when we were still under the Public Health Emergency guidelines, based on a previous lower income, Medi-Cal was not terminating people. All of that has been lifted. If you have verified your income with the county Medi-Cal office, eventually, the adults should transition to Covered California based solely on income.

    • @KevinKnauss
      @KevinKnauss Місяць тому

      It is possible that you were determined eligible for Medi-Cal during the annual open enrollment period when Covered California renewed your health plan for 2024. If your income was below 138% of the federal poverty level, then Covered CA would have flipped you into Medi-Cal. There is no transfer of tax return data between the IRS and Covered Ca. Know one at Covered Ca can see your tax return. Covered Ca solely off of the estimated income in your account. If you have not updated it for a while, it may be low and Medi-Cal range. You must reconcile the subsidies you received from Covered CA on your federal tax return. If this has not happened, you'll be getting a letter from the IRS. You need to call Covered CA and see what they have listed as your income. If it is significantly different from reality, you need to contact Medi-Cal and update the income with them.

I have 2 questions about income and changing plans. 1) How should I report a one time estate disbursement I received in July that counts as supplemental income? Should I divide the total amount by 6 and report the monthly amount for the rest of the year? 2) Am I allowed to change plans after reporting the new income as I will probably lose most all of my subsidies at my new annual income level and want to go to a more affordable plan rather than paying full price for a higher tier? Again, thank you for your videos 😊

    • @KevinKnauss
      @KevinKnauss Місяць тому

      1. If the income is a one-time event, you can report as such in the month you received it. You don’t need to spread it out over the rest of the year. Either way, it will increase your annual income and your subsidies will decrease. 2. If you have an enhanced Silver plan (73, 87, or 94), it will increase to possibly a Silver 70 plan. However, that is the only plan change that can happen with an income change. An increase or decrease of income does not trigger the ability to change the metal plan (Silver to Bronze or from Blue Shield to Kaiser) or switch carriers of the health plan.

    • @KevinKnauss
      @KevinKnauss Місяць тому

      I do not have definitive source on what is counted for Non-MAGI Medi-Cal. In your case, qualifying for Medi-Cal under the Medicare Savings Program (Non-MAGI.) However, I do know that Medi-Cal is focused on your monthly income. Everything I have read points to retirement distributions being counted as income under most circumstances. The wrinkle, that you point out, is whether you take it in a lump-sum (an increase in assets) or in a set monthly distribution. The other issue is that there is some communication between Medi-Cal and Social Security. Social Security verifies income based on filed tax returns. This is how they determine the Income Related Monthly Adjustment Amount if your income is high and whether you qualify for the Part D Low Income Subsidy. In short, if the distribution is taxable, and shows up on your tax return, but you don't report it to Medi-Cal, they would probably have questions about the discrepancy, IF, the distribution put you over the income limit for one of Medicare Savings Programs.
